Longmont United Hospital is proud to be able to offer the COVID-19 vaccination to our flourishing communities, and we recognize that many individuals across the Longmont and surrounding areas are excited to receive the vaccine.

Our Emergency & Urgent Care Center in Frederick is one of our ambulatory vaccine clinics helping to provide the COVID-19 vaccine to our communities in addition to its routine emergency and urgent care offerings. It is located inside the Indian Peaks Medical Center building, just east of I-25 on Highway 52 in Frederick (4943 State Hwy. 52 #100, Frederick, CO 80515).

“Centura Health is currently operating more than a dozen vaccination clinics for our patients and community partners and our commitment is to every community, every neighborhood, every life – whole and healthy,” said Antony Pearson, MD, Chief Medical Officer (CMO), Longmont United Hospital. “We’re fulfilling that commitment one COVID-19 vaccination at a time and are proud to have our Emergency & Urgent Care Center in Frederick as a trusted vaccination location for our Longmont-area community members.”

If you are a Centura Health patient, you will receive an invitation when it is your time to receive the vaccine through MyCenturaHealth, our patient portal. These invitations are sent based on the Colorado COVID-19 vaccine distribution plan. The invitations will be randomized and equitable without discrimination based on race, color or ability to pay.

In Colorado, Centura Health is pleased to currently extend invitations to those in the 70 years and older community to receive their vaccinations for COVID-19. We have launched clinics across our Colorado hospitals and other clinical sites to ensure access for everyone. To receive a vaccine, you must receive an email invitation from Centura Health. Appointments are required; we cannot accommodate walk-ins at this time.

When will I receive my invitation?

If you are a Centura Health patient, you will receive an invitation when it is your time to receive the vaccine through MyCenturaHealth, our patient portal. This process will be randomized and equitable based on weekly vaccine allocation from the state.

In Colorado, if you do not have a MyCenturaHealth account, you may sign up to receive the vaccine using the “Vaccine Notification Sign-Up” button on our website at https://www.centura.org/covid-19/covid-19-vaccine-information.

If you are unable to use the vaccine notification sign up form, please call 866-414-1562.

What is the process when I arrive to receive my vaccine?

You may have your temperature checked and be asked about COVID-19 symptoms when you arrive.

  • Once inside, you will be guided to a socially-distanced seating area where your caregiver will share information with you about the vaccine and will answer any other questions you may have, they will then administer your vaccine.
  • After receiving the vaccine, there will be a 15-minute observation period.
  • State/government-issued identification and social security numbers are not required to receive the vaccine.
  • There will be no charge to receive the vaccine.
